Can you get a PhD with no publications?

The short answer is no. Publications are not required to apply for a PhD. The longer answer is that the admissions committee wants to see that you have the potential to become an excellent researcher. While publications are one indication of this, they are not the only way to show that you have strong research skills.

How many citations are considered good?

What is a good number of citations? With 10 or more citations, your work is now in the top 24\% of the most cited work worldwide; this increased to the top 1.8\% as you reach 100 or more citations. Main take home message: the average citation per manuscript is clearly below 10!

Is it possible to get a PhD by writing a thesis?

There has been a fair bit of, in my view, un-warrented snobbery about this mode of getting a PhD. This, in addition to some confusion about how copyright works, has meant that including whole papers in a PhD thesis, which enables the author to graduate with an already populated resume, is not as popular as perhaps it should be.
